From Conversations to Actions Using the IPP

This booklet shares real life stories of how people with intellectual/developmental disabilities set personal goals and objectives and work through the Individualized Program Plan (IPP) process to achieve them. Highlights include personal journeys, tips about how to have successful team meetings and typical traps to avoid. The booklet is used as a training tool in case management organizations.
